Project Termination Confirmation Form (EXT-4)

Please complete the form below to terminate all animals as scheduled for your project.

ADI requests written instructions for project termination and extensions. All projects will be terminated either on the scheduled termination date or within the next 5-7 working days. We shall attempt to contact you by email/phone. Please remember that it is the researcher’s responsibility to provide written instructions for project termination.
